The River Valley Area Agency on Aging (AAA) and Disability Resource Connection is a valuable resource for seniors, people with disabilities, family members, caregivers and professionals. We provide information and assistance, awareness, and access to help you stay in your home.

 

Information and Assistance

Call 1-800-615-4379. A counselor will assist you in finding services and resources. We have a database of over 25,000 resources statewide.

GeorgiaCares

River Valley AAA administers the state health insurance and assistance counseling program, GeorgiaCares. This program provides personalized counseling and informs people about available Medicare plans, prescription assistance, and open enrollment.

 

Medicaid Waiver Services

River Valley AAA administers the Georgia Community Care Services Program that includes determining eligibility with reliable intake and screening and the provision of care coordination services.

The Community Care Services Program (CCSP) helps people who are elderly and/or functionally impaired to continue to live in their homes and communities by offering an alternative to nursing home placement. CCSP does operate off of a waiting list. Length of time spent on the waiting list varies with individual level of need and funding available.

Options Counseling and Money Follows the Person

River Valley AAA is the local contact agency to start discussions with residents who wish to return to their communities after staying in a nursing home at least 30 days.  Contact our office to learn more about programs that assists residents in resettling in the community with home and community based services.

 

Home and Community-Based Services

River Valley AAA coordinates home and community-based service delivery with its extensive service network providing transportation, home-delivered meals, in-home services, respite care, and homemaker services. 

Home & Community Based Services (HCBS) offer a variety of services to all individuals 60 years and older, at little or no cost.  HCBS does operate off of a waiting list. Length of time spent on the waiting list varies with individual level of need and funding available.

Senior Centers

River Valley AAA funds and/or operates senior centers in the 16-county region. For those in need of additional care and observation, adult day care centers provide much needed respite to caregivers. Click here to view the list of Senior Centers in the River Valley Region.

 

Case Management

River Valley AAA case managers provide timely, in-home assessments of environmental, social and financial needs and develop individualized plans that link clients with community resources.

 

Wellness and Evidenced-Based Services

River Valley AAA provides a wide range of activities and health information to help people get healthy and stay healthy. Click here to view a listing of programs available in the River Valley region and throughout the state.

 

One of the Wellness and Evidence-Based Services provided is the Chronic Disease Self Management Program. The Chronic Disease Self Management Program (CDSMP) helps individuals with chronic conditions, or those you know anyone who has a chronic condition, learn how to manage their conditions effectively in order to live healthier lives. Caregiver Support

River Valley AAA supports caregivers through a variety of means including both in-home and out-of-home respite care, personal support services, adult day care, telephonic care consultations, and caregiver support groups.
